Restaurant inspections


Restaurant inspections are performed in the county by Miami County Public Health, except in Piqua, which has its own health department. These inspection reports were provided by Miami County Public Health.

May 16

Troy Apostolic Temple/Little Jacob’s Ladder, 625 N. Co. Rd. 25-A, Troy — Equipment and/or components are not maintained in good working order. One door refrigerator thermometer reading 48-50F but foods inside are 38F. Repair refrigerator thermometer or obtain a thermometer to place inside for accuracy.

Ulbrich’s Hometown Bakery, 11 Norwich Rd., Troy — Equipment food-contact surfaces and utensils were not being sanitized. No sanitizer was available for use, Worker states she will take trays back to main baker. Obtain sanitizer. The physical facilities are not being maintained in good repair. Observed ceiling tiles with water damage from leaking roof. Critical violation: Observed improper display or storage of poisonous or toxic materials offered for retail sale. Observed chemicals stored on shelf with gloves and other sing e service articles.

May 17

Dad’s Carry Out, 249 Union St., Troy — Observed food employees wearing jewelry on arms or hands during food preparation. Observed cooler condenser dripping water into pans. Observed improper draining of condensation and other nonsewage liquids.

Izzy’s Drive Thru, 7500 St. Rt. 571, West Milton — Observed fishing bait stored in walk-in cooler with drinks. Designate area for bait away from drinks or clean items. Miscellaneous sources of contamination observed. Observed no towels or hand drying device at the hand washing sink(s). Paper towels must be in a holder or dispenser at all times. Clean floor in small walk-in cooler of build-up. Observed a build-up of dirt and debris. Observed back room storage area with unclean floor, shelves, and walls. Focus on debris around shelves and spider webs on walls and shelves. Observed a build-up of dirt and debris. Clean dusty build-up on and around walk-in cooler fan.

Rich Oil, 120 N. Miami St., Bradford — Critical violation: observed hot dog bun ingredient label facing an upright cooler and not readable. Observed packaged product(s), labeled on-site, with a missing and/or incomplete allergen declaration(s). Observed pop machine leaking water down the front. Equipment and/or components are not maintained in good working order. Observed service sink not being used and blocked with items. Observed dust build-up near cooler condenser fan area.

May 18

CVS Pharmacy, 914 W. Main St., Tipp City — Observed milk cooler with unclean surface and absorbent pad that needs changed. Non-food contact surfaces of equipment are unclean. Went through food code changes that involve management training and having a bodily fluid accident plan.

David L. Brown Youth Center, 291 S. Childrens Home Rd., Troy — Critical violation: observed open packages of turkey and ham in refrigerator without date marks. Refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not proper y date marked.

Fayevores Banquet Center, 2334 S. Miami St., West Milton — Discussed changes to the food code.

Heck Yeah, 5795 N. Co. Rd. 25-A, Piqua — There is no test kit available for measuring the concentration of the sanitizer for dishwasher. Critical violation: observed inside of ice machine with unclean residual. Walk-in beer cooler (downstairs) has issues with ceiling and wall panels. Fix, replace, re-secure walls panels properly, The physical facilities are not being maintained in good repair. Observed floor tiles in kitchen area that are in disrepa’r. Replace tiles where needed. The physical facilities are not being maintained in good repair.

La Piazza, 2 N. Market St., Troy — Critical violation: observed the presence of live insects, rodents, and other pests. Observed many gnats in the basement. Water is standing in some areas. A pest control agency must be obtained to remedy the issue. Critical violation: TCS foods were not being held at the proper temperature. Observed cooked pasta being held on counter at 61F. Food discarded. Critical violation: refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked. Observed opened packages of ham and chicken without any date marking. Critical violation: ready-to-eat, TCS food that had been date marked was not properly discarded when required. Observed moldy food in the refrigerator of pantry. Critical violation: no chlorine sanitizer was being used in the dish washing machine. Worker states that she knew that it was out. The concentration and/or temperature of a chlorine sanitizing solution did not meet the minimum requirements specified in the code. Critical violation: equipment food-contact surfaces or utensils are dirty. Observed food debris in the can opener, chopper, and food processor that was indicated as clean. Observed a lot of old food on the slicer and under the slicer. The hand sink in the pantry is draining very slowly, the plumbing system was not properly repaired. Observed light bulbs or heat lamp in the kitchen that were not properly shielded or coated where required. Observed mops being dried using an unapproved method. The mop head was in bottom of utility sink.

Naina Petroleum, 1016 W. Main St., Tipp City — Observed front cabinet storage surfaces unclean in the inside area. Non-food contact surfaces of equipment are dirty. Observed utensil boxes sitting on the floor. Cleaned equipment and utensils, laundered linens, or single-service and single-use artlcles are not properly stored. Observed mop sink faucet leaking water. The p umbing system was not properly repaired. Observed no towels or hand drying device at the handwashing sink(s) at front sink and men’s restroom. Observed dust build-up on fan area in walk-in cooler. Observed a build-up of din and debris.

May 19

Basil’s Restaurant, 18 N. Market St., Troy —

Ccritical violation: Observed food that was not properly protected from contamination by separation, packaging, and segregation. Observed raw meats stored in walkin cooler above or next to cooked ready to eat meats. Corrected during inspection. In-use utensils are improperly stored. Observed the handle of ice scoop behind the bar in the ice. Corrected during inspection. Observed improper storage of food items. Observed pans of raw meat on floor of walk-in cooler in basement. Corrected during inspection. Critical violation: TCS foods were not being held at the proper temperature. Observed foods being held at 48F in a two-drawer refrigerator. All foods removed. Equipment and/or components are not maintained in good working order. A two-drawer refrigerator is holding at 46F.

Igloo Drive Thru, 945 W. Main St., Tipp City — No violations at time of inspection.

Tom’s Pizza, 690 S. Miami St., West Milton — Observed three-compartment sink with a damaged wash compartment sink divider that allows water to go into the rinse compartment. Replace three-compartment sink with a sink that meets code requirements, or repair sink properly and safely. There is no test kit available for measuring the concentration of the sanitizer. Clean inside freezer bottoms of unclean residual.

VFW Pleasant Hill, 7578 Fenner Rd., Pleasant Hill — There is no test kit available for measuring the concentration of the sanitizer. Observed water in the bottom of beer cooler, Equipment and/or components are not maintained in good working order. Observed butter container being reused for food storage. Observed re-use of single-service or single-use articles. Corrective Action(s): Single-service and single-use articles may not be reused. Observed stove and fryer cooking area with food residual build-up. Non-food contact surfaces of equipment are dirty. Wall behind the portable fryer has grease build-up absorbing into the paint. Install a smooth easy cleanable non-absorbing surface on the wall. Floors, walls, and/or ceilings were not smooth and easily cleanable.

May 20

Cliffside Golf Course, 6510 S. St. Rt. 202, Tipp City — Observed food equipment (ex. microwave) that are not approved by a recognized testing agency.

Leaf & Vine, 108 W. Main St., Troy — Observed liquid in bottom of beer cooler. Observed pop gun dispenser nozzle with inside residual build-up.

Piqua Fish & Game, 9344 N. Spiker Rd., Piqua — A thermometer capable of accurately measuring the temperature of thin foods was not available. There is no test kit available for measuring the concentration of the sanitizer.

Troy Senior Citizens Center, 134 N. Market St., Troy — Satisfactory at time of inspection.
