Frickers, 1187 Experiment Farm Rd., Troy — Observed employee drinks being stored near prep area. In-use utensils are improperly stored. Observed plastic scoops stored inside soups in walk-in cooler. Observed improper storage of damp wiping rags. Observed faucet on top of three compartment sink to be leaking water. Observed ice holder bin under drink dispenser accumulating a lot of condensation and dripping under cabinet. Cutting board heavily scratched and scored in front of prep coolers. Observed re-use of single-service containers to hold a large amount os sauces for wings. Critical violation: observed mold on the inside of the ice machine. Observed food residual build-up on handles of walk-in cooler. Single-service or singl-use articles, kitchenware, or tableware were not handled, displayed, or dispensed properly. Observed forks stored inside open cup with their handles down. Observed grease trap under the table next to dishwasher to have and odorous smell coming from it. Observed no towels or hand drying device at the hand washing sinks. Observed standing water on the floor behind the ice machine. Handles on the hand washing sinks were rough to the touch and had residual build-up on them. Critical violation: observed a working spray bottle that once had cleaner in it being used to store water for cleaning without any identification as to what was in the container. Observed personal items being stored on racks next to food contact equipment. Corrected during inspection.
K’s Hamburgers, 117 E. Main St., Troy — Critical violation: observed raw bacon stored over eggs in cooler. Corrected during inspection. Observed dish racks that are no longer cleanable with rust and cracks. Dishwashing machine rinse cycle not functioning properly. Water was not coming out with enough pressure to rinse properly. Observed gaskets in multiple coolers that are coming apart and have built up food residual. Observed food thermometer that was not reading the correct temperature. Observed residual build-up on shake machine, in bottom of freezer, on freezer handles, on the back of the blade guard, and in central cooler. Observed restroom without self-closing door and without hand washing signage. Observed ceiling in dry storage room that is coming down and a panel wall in the same area that is broken and coming about. Observed black residual build-up and rust on FRP behind the dishwasher.
Sakai Japanese Bistro, 2303 W. Main St., Troy — Observed employee drinks being stored on storage rack next to clean items. Critical violation: observed raw shrimp being stored on top of beer and next to ready to eat foods. Corrected during inspection. Observed wet wiping cloth kept out of sanitizer on cutting board. Observed ice cream in walk-in freezer stored on the floor. Critical violation: observed food in reach-in sushi prep cooler in bar area that was not at proper cold holding temperature of 41F or less. Observed sushi roller equipment with a cracked lid. Observed gaskets on prep cooler behind the grill that are coming apart from the equipment and need cleaned. Observed residual build-up around prep sink, on handle and along door of the walk-in freezer, and on handles of coolers and walk-in cooler. Observed employee hat sitting on metal food tray with clean items. Critical violation: observed the air gap between a plumbing fixture and ice machine drainage line that was not sufficient to prevent back siphonage of contaminated water. Critical violation: observed a scotch pad being stored in the hand washing sink. Observed a black residual build-up along FRP wall behind dish area. Observed a build-up of debris on floor under bag-in-box syrups and a build-up of dust on shelving above the dish area and on fan. Critical violation: observed the presence of gnats in dish strage area and near pop syrup.
Meijer, 1900 W. Main St., Troy — Critical violation: observed improper use of time as a public health control for up to four hours. Observed cooked popcorn shrimp and chicken sandwich without time stamp on packaging. Cold or hot holding equipment did not contain a thermometer or the thermometer was not correctly placed. Observed condensation dripping from ceiling and condensors in the deli freezer and meat room freezer.
Walmart, 1801 W. Main St., Troy — Observed ice build-up in walk-in freezer in meat department. The surfaces of cutting baord were severely scratched or scored and could not be effectively cleaned. Critical violation: observed food residual on top interior of microwave. Corrected during inspection. Observed residual build-up on handles of equipment in bakery area, on chicken storage carts in deli walk-in cooler, and on doughnut glazing machine in bakery area. Observed employee coats and hats being stored on shelving with clean items. Critical violation: observed pallet of itemsin front of handsink in produce area, making the hand sink not easily accessible.
West Milton IGA, 1177 S. Miami St., West Milton — Observed food in packaging that does not appear to protect it from contamination. Observed dented cans in storage area, one of the cans was covered in rust. Observed improper storage of wiping cloths, stored directly on stainless steel surface behind deli counter. Critical violation: TCS foods were not being held at the proper temperature. Observed whole roasted chickens being stored in hot holding equipment that was not holding the internal temperature of the chicken correctly. Corrected during inspection. Critical violation: refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked. Observed pepper stuffing that was in the walk-in cooler that was dated over a month before the day of inspection. Corrected during inspection. Critical violation: observed packaged candy on bottom bins on entire bottom rack close to the entrance of the store, labeled on-site, with a missing ingredient and allergen declaration. Equipment no approved by recognized testing agency: roaster being used to cook potatoes, residential microwave, non-food grade containers in the walk-in cooler. Observed heavil scored cutting boards in meat prep. Critical violation: food residual on the ceiling of the microwave. No hand washing signage in employee restrooms.
Mr. Deal’s Supermarket, 689 S. Miami St., West Milton — All previous issues have been resolved.
Troy Bulk Barn, 4 W. Main St., Troy — Observed employee drink stored in deli display case. Critical violation: observed food that was not properly protected from contamination by separation, packaging, and segregation. Raw sausage and eggs were stored over ready to eat foods in prep cooler. Observed water leak at hand sink and prep sink faucets.

Susie’s Big Dipper, 323 N. Main St., Piqua: Milk in single door refrigerator at 48 degrees. Keep all TCS foods below 41 degrees to prevent bacterial growth. Thermostat lowered. Monitor closely. Ice machine interior needs to b e cleaned.
Rally’s, 118 N. College St., Piqua: Milk shake machine holding at 57 degrees. Keep all TCS foods below 41 degrees to prevent bacterial growth. Machines taken off line. Repair called in. Observed broken food containers. Food containers must be smooth and easily cleanable. Discard. Breaks provide areas for bacterial harborage. The fryer area and warmer need to be cleaned. The floors need to be cleaned.
Uniqueworks, 8433 N. County Road 25A, Piqua: The chest freezer has a damaged lid. Equipment must be kept in good condition and intact. Repair/replace. Replace missing ceiling tiles to prevent possible contamination. Walls in frying area of kitchen have not been completed. Complete so walls are smooth, durable and easily cleanable.

Burger King, 1408 Covington Ave., Piqua: Vegetable cutter and tomato cutter on floor in dish area. Keep equipment six inches off floor to prevent possible contamination. The following items need to be cleaned: floors, ceilings, warmer cabinet, shelving in storeroom, mop sink.
Subway, 1300 E. Ash St., Piqua: Satisfactory at time of inspection.
Kids Learning Place, 285 R.M. Davis, Piqua: Satisfactory at time of inspection.
JC Mulligan LLC, 110 W. High St., Piqua: Observed food employee serve ready to eat food with bare hand. Cannot have bare hand contact with ready to eat items. Observed roast beef, 56 degrees, and turkey, 48 degrees above 41 degrees in prep unit. Must keep TCS foods below 41 degrees while prevent bacterial growth. Containers were too full. Top layer discarded. Observed ribs, 45 degrees, and chicken soup, 48 degrees in walk in cooler. Keep these TCS items below 41 degrees to prevent bacterial growth. Repair called in. Observed cracked food containers. When food containers are cracked the cracks provide areas for possible bacterial harborage. Should be smooth and easily cleanable. Discard. The following items need to be cleaned: shelves, hood, floors by dishwasher, grease trays and ceiling by dishwasher.
Faith’s Piqua, 536 Wilson Ave., Piqua: The dishwasher was not sanitizing properly. Must dispense 50 ppm of chlorine. Repair called in. Wash, rinse and sanitize in three-compartment until repaired. Back door was open without a screen. Openings should be secured or screened in to prevent possible pest entry. The following items need to be cleaned: dough roller, ceiling, floors, mixer, dishwasher, upright freezer, metal stem thermometer. Repair paint above pizza oven so smooth.
Arby’s Roast Beef, 1230 E. Ash St., Piqua: Observed open container of buns next to hand sink. Keep food away from possible source of contamination. Corrected. The hot holding area needs to be cleaned.
