Dec. 6

Holiday Inn Express, 60 Troy Town Dr., Troy — Refrigerator in dining area did not have a thermometer. Observed food debris in bottom of left freezer. Observed a build-up of food residual underneath freezers.

Speedway, 818 N. Market St., Troy — Critical violation: person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report their health information. Observed gasket on walk-in cooler door inside beer cave completely falling off the door. Critical violation: equipment food-contact surfaces or utensils are dirty. Observed residual build-up on the top of the microwave toward the back. A few of the soda nozzles by the fountain drink machine were found to have slight build-up underneath. Observed no hand washing signage in men’s restroom.

Dairy Queen, 513 W. Main St., Tipp City — Critical violation: person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report their health information. Person in charge did not ensure that the facility has written procedures of vomiting and diarrheal events. Repeat observation: observed two-door reach-in freezer with ice build-up issue. Cardboard was placed on the first shelf to prevent potential contact with food. Critical violation: observed interior top of microwave with built-up food residual. Corrected during inspection.

Dec. 7

25-A Cafe, 1469 S. County Rd. 25-A, Troy — Observed employee drink sitting on shelf above prep cooler. Observed dented can in back of kitchen prep facility. Corrected during inspection. Observed wiping clothes being stored in sanitizing bucket without any proper sanitizer. Corrected during inspection. Observed leak in faucet above three compartment sink. The surfaces of cutting boards are heavily scored and need to be resurfaced or replaced. Observed residual on the handles of most of the equipment inside of the prep kitchen. Observed old food residual build-up in the wash basin. Observed a cell phone, keys and other personal items laying on top of cutting boards.

Arbogast Buick Pontiac GMC, 3540 S. County Rd. 25-A, Troy — The surface of the cutting board in front of the prep cooler was heavily stained and scratched and needs to be resurfaced or replaced. Observed coffee stirrers being stored within customer reach without protection. Corrected during inspection.

Crystal Room, 845 W. Market St., Troy — Observed food in packaging that does not appear to protect it from contamination. Observed dented cans in storage rack. Observed improper use and/or maintenance of wiping cloths. Wiping cloths stored in dirty sanitizer water that had very low concentration of sanitizer. Observed cutting board in front of food prep table that is heavily scored. Observed items laying on top of plastic forks and spoons. Observed no supply of hand cleaning liquid, powder, or bar soap at the hand washing sink. Observed no paper towels at the hand-washing sink. Observed no hand washing signage.

Elizabeth Twp. Community Center, 5760 E. Walnut Grove Rd., Troy — Person in charge has level two training in food safety. Discussed pest control and advised executive director to ensure that pest traps are enclosed. No violations at time of inspection.

Monroe Twp., 4 E. Main St., Tipp City — Critical violation: person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report their health information. Person in charge did not ensure that the facility has written procedures of vomiting and diarrheal events. Repeat observation: observed residential refrigerators. Equipment is not approved by a recognized testing agency.

Speedway, 1475 W. Market St., Troy — Critical violation: person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report their health information. Person in charge did not ensure that the facility has written procedures of vomiting and diarrheal events. Observed build-up of food residual on prep cooler door and handle. Observed sticky syrup residual on floor in back stock area near pop boxes. Observed a build-up of debris underneath shelving in beer cooler. Observed residual build-up in hard to reach areas, such as in the corners or behind equipment.

Dec. 8

Hickory River Smokehouse, 135 S. Garber Rd., Tipp City — Observed a residual build-up under shelving in dry storage. Manager and owner have level two training. The facility has all of the required procedures. FAcility is very organized and clean.

Outback Steakhouse, 1801 Towne Park, Troy — Observed employee drink sitting on shelf above prep cooler. The surfaces of cutting boards are heavily scratched and need to be resurfaced or replaced. Observed mold growth on walls of drinking glass holding units in the bar area. Faucet in the hand sink in the dishwashing area was dripping. Observed missing grout in sections on the tile kitchen floor.

Sam & Ethel’s Restaurant, 120 E. Main St., Tipp City — Observed employee drink on prep cooler cutting board. Observed wet wiping cloth on cutting board. Observed boxes of food being stored on the floor of the walk-in cooler and walk-in freezer. Observed a build-up of dirt and debris along floors and underneath equipment.

Taco Bell, 985 W. Main St., Tipp City — Critical violation: person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report their health information. Person in charge did not ensure that the facility has written procedures of vomiting and diarrheal events. Observed areas on kitchen floor with low grout that are collecting water. Repeat observation: observed a build-up of dirt and debris on counters, under equipment, and floors, especially where the floor and walls meet.

Troy Country Club, 1830 Peters Rd., Troy — Observed employee drink being stored in food prep area next to prep equipment. Observed dented cans in the back storage area of the facility. Observed in-use cup being stored inside of prep cooler in blue cheese bin. Surfaces of cutting boards were heavily scratched and need to be resurfaced or replaced. Observed leak from hot water handle on the faucet above the three compartment sink. The ventilation system was not being properly cleaned. Observed a lot of debris build-up in the hood of the ventilation system above cookers. Observed maintenance tools improperly stored with food equipment that could result in contamination.

Dec. 9

Troy Eagles, 225 N. Elm St., Troy — Observed improper storage of wet wiping cloths. Damp cloth rag was laying on food prep table. Critical violation: refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date-marked. Manager threw away food. Manager was not able to show any test device for the sanitizing solution. The surfaces of cutting board on the prep cooler was heavily scratched and stained and needs to be resurfaced or replaced. Observed re-use of single-service container holding food items that were not originally in the packaging. Critical violations: observed food residual build-up on the top of the inside of the microwave.Observed food residual build-up on most of the handles of equipment. Observed no hand washing signage posted in men’s restroom.
