Monroe Township at 4 E. Main Street, Tipp City - Observed residential refrigerator. All food equipment must be health food code approved. NOTE: Ensure a thermometer is in both refrigerators, cold holding 41 degrees or lower.

Ducky’s at 650 N. Co. Rd. 25A, Troy - Ice cream machine not working. Equipment is not permitted to be used until Health District gives written approval. Base wood has been painted and cabinets are for use this season only. Prior to next season wood cabinets must either be removed or wrapped in PRP board. Remove tape that was used to seal openings and use silicate caulk. Correct within two weeks. Ice blocks are coming from Hare City Ice. No care molding. Install rubber base within two weeks. Licence assured, okay to operate. Observed cloth curtain being used. Remove cloth curtains.

Milton Union Rec. Concession Stand at Potsdam Rd. Toury Field, West Milton - Ensure a working metal-stemmed thin tipped thermometer is present at all times. Note: A thermometer is needed in refrigerator that hold food. Closing date is July 14th. Note: A person in charge on side must have an approved certificate in food safety.

Walmart #1410 at 1801 W. Main St., Troy - Complaint: Customer purchased a raw Bobs Evans spicy sausage roll that had human hair in it. This sausage roll is prepackaged. Review: Went through complain with Walmart. Also told Walmart that this complain has been forwarded to OHIO Department of Agriculture. Walmart does not open this product, they only hold this product prepackaged.


Troy Meat Shop at 502 Garfield St., Troy - Follow up - Drove by facility to observe trash area. Additional trash can had been purchased. Trash area is clean and no trash noticed on ground.


Miami Shores Golf Course at 402 E. Staunton Rd., Troy - Dish machine not on at time of inspection. Ensure machine is reaching final rinse temperature of at least 180 degrees. Temperature acceptable at time of inspection.


Speedway at 818 N. Market St., Troy - Can door found at 125 degrees. Keep hot food at 135 or above to reduce harmful bacteria growth. Corrected. Mop sink area unclean. Clean area. Missing ingredient information on self serve donut. Provide information. Dusty vent covers in walk-in coolers. Clean vent covers. Floors in food service unclean. Clean floors.

Tipp City Food Town at 960 W. Main Street, Tipp City - Complaint: Employees in Deli and Meat Departments are not wearing hair nets. Employees smoking in front of the business. Hair was found in potato salad. Review: West through hair restraint rules with person in charge. Handouts given on hair restraints for employees. Food code requites hair to be effectively restrained, keeping hair from contact with food of food contact surfaces. Also reviewed smoking part of complaint with person in charge.


Noahs Ark Kids Academy at 6789 St. Rt. 41, Covington - Complaint: Musty smell in facility. Upon inspection, the following was observed: Director reports recent rain cause flooding in parking low and grass area. Approximately five inched of water entered facility. Carpets have been professionally shampooed. All water is gone and no odor present at time of inspection. Director reports mats have been thoroughly cleaned with has harboring some of the musty smell. Due to flooding and that water sample needs to be taken again by 6/18/14. Facility must provide new/current water sample within one week. Notice well in parking lot which was recently flooded. Well cap may have been covered with water. Previous inspection requires operation to pull water sample within 1 week. Recommend facility follow boil alert procedure until water sample results are in. Will send boil alert guidelines.


Taco Bell at 985 Main St., Tipp City - Follow up - Spoke with General Manager who reports air condition unit is scheduled to be replaced on 6/13/14. Temperature on cold prep line found at 32 and 46 degrees for pico, cheese 35 degrees, 36 degrees, 39 degrees. Foods were on ice at time of inspection. Manager reports foods are still discorded every 4 hours. Bottom portion of cooler holding foods at 35 degrees. Will follow-up after new unit is installed. Kitchen area extremely hot due to air conditioning not working. Keep feels on ice and discard foods and change out serving pans and utensils every 4 hours per Taco Bell Standards.


June 4


McDonald’s at 127 N. Miami St., West Milton - Water leak at hot water heater. Repair to prevent leak. Damaged/missing grate at water heater. Compartment sink area. Repair area and install vent. Noticed odor by drains. Ensure grease trap was recently cleaned. Missing ceiling rent covers. Replace covers. Food prep sick drain unclean. Clean drain area. Found pop area unclean including cabinets. Clean area. License assured due to new ownership.


Kidz Town Learning Center at 2464 Peters Rd., Troy - 30 day - Floor is being replaced soon. At next inspection, all person is charge per shift must be certified in a level one food certification. Plumbing permit for water heater and back flow certification must be done within 2 weeks.


June 5


Leaf and Vine at 10815 Main St., Troy - Clean inside beer coolers when needed. Observed liquid residual in coolers.


Duke Park Concession Stand at 1670 Troy-Sidney Rd., Troy - Food service in good condition at time of inspection.
