Fazoli’s #1715, 1887 W. Main St., Troy — Critical violation at time of inspection: Kitchen temperature is at more than 100 degrees Fahrenheit. This is causing prep coolers not to hold 41 degrees Fahrenheit or below. Two prep coolers are holding food between 45-48 degrees. The larger prep cooler has condensation water in the bottom. So service both prep coolers properly and ensure food maintains 41 degrees Fahrenheit or below to limit growth of bacteria. All prep cooler/refrigeration is not designed to operate and maintain food at/below 41 degrees in a room temperature of 100 degrees or above. Fix ventilation/air conditioning issue immediately. Management must time mark all temperature-controlled for safety foods (TCSF) until issue is taken care of. Meaning use or dispose of food within four hours. This is a critical issue and must be taken care of immediately. Observed small section along oven wall with stand water in grout line section. Regrout this section to prevent water collection in this area. Ensure oven and oven area surfaces (ex. wall) is cleaned when needed. Observed food residual in this area.
Fraternal Order of Eagles, 715 E. Broadway St., Covington — Facility currently under boil advisory. Handouts given for hand washing, ice making, cleaning and sanitizing. Do not operate the kitchen or ice machine until portable water can be purchased.

Steak n Shake No. 260, 1779 W. Main St., Troy — Critical violation at time of inspection: observed food debris on can opener. The pan holding clean soup spoons has food debris in bottom. The milk dispenser has an accumulation of milk splash on sides. The shake machine has food debris splash on sides also. Equipment must be kept clean to prevent contamination. The handles on ice cream freezer and refrigerator are sticky to touch and unclean to sight. Clean more often. The shelf under the grill continues to have accumulation of food debris such as grease/egg shells after food debris. REPEAT. Observed a trash can sitting next to shelf of clean dishes. Food debris from trash can had splashed onto clean dishes. Remove trash can from area. DO NOT store near clean dishes. Observed a leak from sewage pipe under dish machine. A foul odor is coming from area. Also: fix pipe and clean area. Observed a lot of food debris and items under shelves and hard-to-reach areas. Critical violation at time of inspection: observed a lot of gnats near drains in kitchen. Consult pest control and clean areas thoroughly. Discussed the need of dishwasher to eliminate any contamination to dishes or food from servers.

Fazoli’s No. 1715, 1887 W. Main St., Troy — Follow-up inspection for air conditioning unit. It is being replaced Monday. Remember when temperature of prep cooler/foods rise above 41 degrees Fahrenheit for all TCFS. Also, all TCFS must be time marked and used or discarded in four hours to ensure limited bacterial growth. Also, after unit is fixed, ensure maintenance on prep cooler is needed.

Fletcher Pizza Carry Out, 101 W. Main St., Fletcher — Critical violation at time of inspection: Observed sausage, ham, turkey in two-door refrigerator without any date marking. All opened packages of lunch meat, pizza topping, cut tomatoes and greens must be dated and used within seven days if not used within 24 hours. Critical violation at time of inspection: observed equipment with dried food debris. Clean equipment after use or every four hours. Observed many containers of oil in back of facility. The containers must be removed in a timely manner. Critical violation at time of inspection: observed mouse droppings in food prep area. The area must be cleaned and work must be done to eliminate the pests. Observed items in back room being stored not being used. Remove any unnecessary items to facilitate easy cleaning.
